Job Overview

Slough Borough Council is seeking a Complex Needs Navigator to support the Rough Sleeping Team in delivering intensive support to individuals experiencing or at risk of repeat homelessness.

The role focuses on working with a small group of ex-rough sleepers who have complex needs and require coordinated support from multiple services. The successful candidate will provide person-centred support, advocacy and practical assistance to help clients access housing, health, welfare and employment pathways.

This role requires a proactive approach, the ability to build trust with vulnerable individuals and experience working within multi-agency environments supporting people with complex needs.

Key Responsibilities

Deliver person-centred support to individuals with complex needs and a history of rough sleeping
Build trusting relationships with clients and motivate positive behavioural change
Develop and implement focused support plans in partnership with other agencies
Support clients to access accommodation, health services, substance misuse services and welfare benefits
Work collaboratively with partners including housing teams, mental health services, drug and alcohol services and voluntary sector organisations
Conduct outreach sessions and engage with rough sleepers in the community
Assist with delivery of services such as night shelters, SWEP and rough sleeper counts
Maintain accurate case records and produce high quality casework reports
Attend multi-agency meetings and case conferences to coordinate support
Ensure all work complies with safeguarding, health and safety and GDPR requirements

Essential Experience & Skills

Experience working with rough sleepers or individuals experiencing homelessness
Experience working within outreach services, housing advice, homelessness services or tenancy sustainment
Strong understanding of complex needs including mental health, substance misuse and social exclusion
Experience delivering person-centred support planning and case management
Experience working in a multi-agency environment supporting vulnerable clients
Strong communication, advocacy and relationship-building skills
Ability to work independently and manage complex caseloads

Additional Requirements

Ability to undertake outreach work including early morning or evening shifts where required
Ability to use a personal vehicle for business use
Strong IT skills including Microsoft Office and case management systems
